Dental Wellness Plan

Exclusions and Limitations

• This program is a discount plan, NOT a dental insurance plan.
• It is ONLY ofered to patients with NO dental insurance.
• Lab Fees are excluded in this plan

The Peak DentaL Wellness Plan cannot be used:

• In conjunction with another dental plan
• For cost of dental care that would be covered under automobile medical/ dental claims
• For services or injuries covered under worker’s compensation
• For treatment facial esthetic procedures such as Botox, Fillers or PRP Facials.
• For dental products such as teeth whitening gels and toothbrushes

Terms and Conditions

Renewal Conditions: By joining the wellness plan, you are authorizing Peak Dental to bill your credit card or checking account for the plan. This charge shall remain in force until you notify Peak Dental of your request to cancel. By joining, you indicate you have read the terms and conditions of the plan. This plan will
automatically renew at the end of our membership term on an annual basis, and your credit card or bank account will be automatically charged or drafted for the appropriate amount.

Termination Conditions: Peak Dental reserves the right to terminate plan members from its plan for any reason, including non-payment.

Cancellation Conditions: You have the right to cancel within the first 30 days as long as the plan is not utilized within the cancellation period. If the plan is utilized within the cancellation period and discounts are received, the cancellation period is void, and no refunds will be given. There is a 30-day notice period prior to your anniversary date to cancel the plan.
